About Us

At MSOLTS, we provide practical, consistent, and person-centred NDIS supports to people living with complex mental health challenges and diverse life circumstances.

We work alongside participants to build routine, stability, and independence in everyday life. Many of the people we support have experienced trauma, social disconnection, or significant barriers to community participation, including those living with psychosocial disability, acquired brain injury (ABI), alcohol and other drug (AOD) challenges, and chronic health conditions.

About the Role

We are looking for a grounded, reliable, and experienced Community Support Coordinator to join our team.

This role combines direct support with coordination responsibilities including reporting, rostering, staff mentoring, and supported transport.

What You’ll Be Doing

  • Supporting participants to build daily routines and independence
  • Providing psychosocial and functional support
  • Assisting with community access and engagement
  • Coordinating supports aligned with NDIS plans
  • Maintaining reports and documentation
  • Supporting rostering and continuity of care
  • Providing guidance to team members
  • Liaising with families and allied health providers
  • Providing safe transport support

Who We’re Looking For

  • Certificate IV in Community Services (minimum)
  • Diploma or higher qualification highly regarded
  • Experience with complex mental health and psychosocial needs
  • Understanding of trauma-informed practice
  • Strong communication and organisational skills
  • Knowledge of the NDIS

Mandatory Requirements

  • Current driver’s licence
  • Access to a reliable, comprehensively insured vehicle (where required)
  • Willingness to drive company vehicles
  • NDIS Worker Screening Check (or ability to obtain)
  • Working With Children Check (WWCC) (or ability to obtain)
  • National Police Check (or ability to obtain)
  • Completion of NDIS Worker Orientation Module (or willingness to complete)
  • Ability to provide required identification documentation

Employment Conditions

  • Full-time role with probation period in line with Fair Work standards.
  • Ongoing performance review and feedback apply.
  • Commitment to MSOLTS as primary employment is required to support continuity of care.
  • Leave entitlements provided in accordance with the National Employment Standards and SCHADS Award.

Salary & Award

This position is classified under the SCHADS Award Level 5.
Remuneration is above the minimum award rate and aligned with Market Salary Rate requirements.
