North Western Melbourne Primary Health Network has been established to improve the health of people who live in the region, particularly those most at risk of poor health outcomes. The Service Navigation Project Lead is a new role created to address accessibility for people with mental health, psychosocial and alcohol and other drugs concerns by improving collaboration, coordination and improved pathways between health services and systems.

This will include:

  • Developing significant understanding of the needs of diverse communities across the region that will benefit from the development of more integrated responses
  • Working with stepped (staged) care intake services to develop standardised approaches for intake, assessment, treatment, and care coordination
  • Establishing coordinated referral processes and pathways to support equitable access to required treatment and support
  • Actively engaging health professionals including GPs, commissioned service providers and other key stakeholders

We're looking for someone with:

  • Proven project management skills such as quality planning, risk management, communications, scheduling and budgeting
  • Understanding of best-practice project change or quality management within the health sector
  • Advanced interpersonal and relationship building skills, with an evidenced capacity for stakeholder engagement
  • High level organisational and time management skills, including the ability to prioritise workloads, work well under pressure, and organise own work and others to meet deadlines
  • Prior experience in the scoping and delivery of major projects within the health, mental health or community sectors (desirable)
  • Relevant tertiary qualification in public health, project management, mental health, community services, or health service management
