NWMPHN has partnered with the Victorian Department of Health and Human Services to lead place-based Suicide Prevention Initiatives (Macedon Ranges and Brimbank Melton) in the NWMPHN region. Each of the trials is supported by governance groups comprising a range of key local participants, with strong representation from community members and people with lived experience of suicide.
The Program Officer, Suicide Prevention will use strong engagement skills and project management capability to build on and drive the successful delivery of a locally developed action plan. The role will also work with the Manager and other teams at NWMPHN to ensure the broader suite of suicide prevention activities and commissioned services are informed by the learnings from this trial site activity and engagement.
We are North Western Melbourne Primary Health Network (NWMPHN), Victoria's largest independent, locally governed and run, not-for-profit PHN, dedicated to improving primary health care in our community.
PHNs have been established with the key objectives of increasing the efficiency and effectiveness of medical services for patients, particularly those at risk of poor health outcomes; and improving coordination of care to ensure patients receive the right care in the right place, at the right time.
