About our Transitional Support Unit (TSU) at Monash Health

Monash Health is the largest public health service in Victoria, employing over 19,000 people across a range of specialities.

The TSU is a community based service, which will be a transition point for consumers between an acute setting and community, or community and the more intensive supported environment of the TSU. Clients eligible for the TSU are also likely to be eligible for the National Disability Insurance Scheme (NDIS).

Operating on a model of least restrictive care, including reduction in the role of chemical restraint in addressing behaviours of concern, we will incorporate a co-design approach in the delivery of activities of daily living. Each of our clients will work in partnership with the occupational therapist and peer worker in designing relevant activities (i.e. groups) consistent with their behavioural support plan.

We are now looking for an experienced Registered Psychiatric Nurse to join our team in an ongoing, full time role.

About the Role

Our multi-disciplinary team support clients within their goals of recovery and transition into function within our community. You will be involved through team discussion within a clinical perspective to advocate for our patients and best practise. With an open approach to group activities and outings into the community, we welcome new ideas and growth to best support our clients.

This is an ongoing, full time role, working 76 hours per fortnight, across a 24/7 rotating roster including AM, PM and night shifts.

About You

You have experience in Mental Health nursing. You may already have expertise in working with consumers who have an Intellectual Disability and/or consumer with a neurological impairment e.g. acquired brain injury (ABI) or traumatic brain injury (TBI), although this is not mandatory. More important is your genuine interest in supporting clients in this specialist area as we will work collaboratively with you to develop and maintain any existing skills gaps you may have.

You are a recovery focused mental health nursing professional with insight into the recovery model of care. You have a keen interest in dual disability and a focus on recovery with individual care planning. To be considered for this role, you need:

  • current AHPRA registration
  • have a qualification in Registered Psychiatric Nursing
  • completed OR currently enrolled into a Post graduate course within Mental health
  • working rights within Australia
