Your Opportunity

Northern Sydney Homelessness Services provides case management and advocacy support to young people (12-24 years old) who are experiencing homelessness or at a risk of being homeless. The case manager will provide client centred support to explore housing options, address mental health/AOD concerns whilst ensuring clients are connected within the community for ongoing independence and stability.

Your key responsibilities will be to:

  • Respond to referrals to the service, and conduct thorough assessments to determine their eligibility and support needs and establish a case plan with the client and the other relevant stakeholders
  • Develop, implement and review individualised case plans for each client, addressing such issues as living skills, welfare needs, interpersonal skills, health, education, training, employment options, social skills, recreational needs and family relationships
  • Provide specialist advice and share knowledge of local networks and maintain positive internal and external stakeholder relationships
  • Develop quality relationships with a range of key stakeholders including Government and community organisations, Community Housing providers, allied health services and other professionals to allow for the effective provision of support and information to clients
  • Support in a range of program activities including the completion of internal and external reports, statistics, feedback summaries and yearly outcomes.

Requirements for Success

  • Relevant degree or diploma with at least 2 years' experience working with young people
  • Demonstrated ability to provide specialised or complex therapeutic interventions and manage a caseload of complex Consumer.
  • Experience with clients dealing with issues including homelessness, poverty, abuse, mental health and the use and misuse of alcohol and other drugs, and knowledge of support services
  • Highly developed administrative skills along with well-developed written and oral communication
  • An understanding from combined lived experience and professional development about factors that support recovery and a proven ability to use this to positively support others on their recovery journey

Before starting work with us, you will need to undertake a national police history check, qualifications and referee checks, and have a clearance to work with children/vulnerable people.
