We are seeking a motivated person who has a genuine approach and proven ability to support people with mental health challenges. This role involves assisting people to implement and coordinate their NDIS plans to build the way to a better life.

The position is Full Time on a fixed term, 1 year contract on a Level 4 on the SCHADS Award. The position is based out of our Rockingham Office

Your role will help us make a difference by:

  • Being adaptable and providing a combination of Support coordination to best meet the needs of the people.
  • Helping people coordinate their NDIS supports and support them to navigate the NDIS environment.
  • Helping people understand their NDIS plans.
  • Working with people to link in with service providers of their choice to receive supports.
  • Working with people to develop safety plans and safeguards to ensure their safety in any times of crisis.
  • Building positive working relationships with clinical and other mental health services and other government and community agencies to support people in a holistic way.

  • You have the ability to build rapport with a variety of people in short period of time
  • You understand the NDIS
  • You have sound self-management skills to prioritise, account for and manage time effectively,
