Job Description

The Care Finder Program is a consortium comprised of Micah Projects, Communify Queensland and Queensland Positive People that supports people who require intensive assistance to interact with My Aged Care, access aged care services and connect with other relevant supports in the community. The program aims to improve access to aged care, strengthen connections to services, and improve integration between health, aged care and community support systems.

We are looking for a compassionate and proactive Aged Care Navigator to work as part of Inclusive Health Partnerships, advocating for and supporting individuals to access appropriate resources, opportunities and aged care services within the community. Through assertive outreach, assessment, support coordination and relationship building, you will help participants navigate the aged care system and remain connected to the services they need.

This is a part-time (4 days/week) employment opportunity at Level 4 of the SCHADS Award.

Desired Skills and Experience

Key Responsibilities

• Assist individuals to understand My Aged Care and the support opportunities available to them
• Conduct assertive outreach to engage and build rapport with potential participants
• Undertake holistic assessments to identify needs and access appropriate supports
• Establish and maintain relationships with organisations to improve access to aged care, specialist services and community resources
• Maintain participant engagement throughout the duration of support and provide assistance until services are in place
• Work collaboratively with service providers and partner organisations to ensure participants receive coordinated support

Essential Criteria

• Relevant Human Services certificate, diploma or tertiary qualification and extensive experience, or a combination of experience, expertise and competence
• Demonstrated knowledge and experience working with people who have experienced disadvantage
• Understanding of assertive outreach and trauma informed care
• Demonstrated ability and commitment to work collaboratively within a team and with external stakeholders

Desirable Criteria

• Previous outreach experience working in a community-based organisation
• Knowledge of the aged care system
• Experience supporting people to navigate complex service systems
