Micah Projects is a community based not-for-profit organisation with a vision to create justice and respond to injustice at the personal, social, and structural levels in church, government, business, and society. Micah Projects provides a range of support and advocacy services to individuals and families.
The Care Finder Program is a consortium comprised of Micah Projects, Communify Queensland and Queensland Positive People that supports people who require intensive assistance to interact with My Aged Care, access aged care services and connect with other relevant supports in the community. The program aims to improve access to aged care, strengthen connections to services, and improve integration between health, aged care and community support systems.
We are looking for a compassionate and proactive Aged Care Navigator to work as part of Inclusive Health Partnerships, advocating for and supporting individuals to access appropriate resources, opportunities and aged care services within the community. Through assertive outreach, assessment, support coordination and relationship building, you will help participants navigate the aged care system and remain connected to the services they need.
This is a part-time (4 days/week) employment opportunity at Level 4 of the SCHADS Award.
