Merri Outreach Support Service (MOSS) is currently seeking applicants for the position of Outreach Case Manager in Broadmeadows. This full-time role is available for 12 months.

MOSS offers a range of services to homeless and marginalised individuals, including men, women, and children, in the North and West Metropolitan Region. Our agency is dedicated to delivering holistic team case management to a diverse and high-needs cross-target group.

As an Outreach Case Manager, your primary responsibility will be providing case management to individuals and families facing homelessness or at risk of becoming homeless. This will be done through a team case management approach, where all team members contribute to the case plan and ongoing support for the consumer. Our goal is to enhance consumer outcomes by providing additional support alongside the existing services offered by Access Points under the Opening Doors Framework.

We are seeking someone with excellent case management skills, and familiarity with the Homelessness sector would be advantageous. At MOSS, we have a supportive and positive work culture, and we offer generous benefits as per the EBA.
