Based on the Mornington Peninsula, with offices in the Casey area and the outer Eastern suburbs, Mentis Assist provides support for people with diagnosed and enduring mental illness.

Community Mental Health Practitioners will provide recovery oriented outreach psychosocial supports to people experiencing mental illness. As a Community Mental Health Practitioner, you will be working closely with mental health clinicians to ensure seamless transition to the EIPSR program and provide ongoing community based support. Applicants must have experience in providing trauma informed practice to provide a framework for assessment and planning of services. This role will also support eligible clients to apply for the National Disability Insurance Scheme (NDIS).

The position is based in Frankston.

Appointment to a role will not be undertaken until the successful completion of reference checks, a National Police Check, Disability Worker Exclusion Scheme Check, and Working with Children Check.
