About the role

Maweja Super Care Pty Ltd is seeking a Community Worker to deliver structured community access and social participation supports to NDIS participants and community clients. This role focuses on helping individuals engage with their community, access services, build independence, and achieve personal wellbeing goals. The position is central to our disability and community services model and aligns with professional welfare and community support practice.

About the service

Our Community Workers provide one on one and ongoing community based support seven days a week. Supports may be delivered as single appointments or as part of an ongoing program, depending on participant goals and needs. Services are provided in community settings and clients’ homes and are designed to promote inclusion, confidence, and independence.

Key responsibilities

  • Interviewing participants and assessing their needs, strengths, barriers, risks and goals relating to community participation, independence and access to services.
  • Planning, developing and delivering support programs and community access activities aligned with each participant’s NDIS plan goals.
  • Assessing community resources and identifying suitable recreational, social, cultural, training, welfare and support services available to participants.
  • Supporting participants to access community based programs, local services, social activities, group programs, libraries, gyms, pools, day programs, faith communities and local events.
  • Supporting participants to build practical life skills, communication skills, social confidence, decision making ability and independence in community settings.
  • Assisting participants to access essential community services such as appointments, shopping, banking and public transport as part of planned community participation and capacity building goals.
  • Liaising with families, carers, support coordinators, allied health professionals, community groups, welfare agencies and service providers to coordinate appropriate participant supports.
  • Providing information, guidance and warm referrals to relevant community, welfare, health, training and support services.
  • Monitoring participant progress, wellbeing, participation outcomes and barriers to engagement, and reporting changes or concerns through the correct internal channels.
  • Preparing and maintaining accurate case notes, progress records, activity reports and participant outcome summaries to support service review and NDIS related reporting.
  • Promoting participant dignity, choice, control, independence and safety while supporting participants to engage with community resources and services.

About You

We're looking for someone with both the formal background and the lived skill set to do this work well.

Qualifications & Experience

  • An AQF Diploma or Advanced Diploma in Community Services, Disability, Welfare, Mental Health, Social Work, or a closely related field — OR at least three years of equivalent relevant experience in lieu of formal qualifications.
  • A minimum of two years' demonstrated experience supporting people with disability or complex needs in community and/or residential settings.
  • Working knowledge of the NDIS, the NDIS Code of Conduct, and the NDIS Practice Standards.
  • Familiarity with SIL service delivery, individualised support planning, and goal-based progress reporting.

  • Strong assessment, planning, and problem-solving skills — you can read a situation, think on your feet, and adjust the plan when life happens.
  • Confident, respectful communication with participants, families, and the wider support team.
  • Patience, emotional steadiness, and genuine respect for participant choice and control — including when those choices differ from your own preferences.
  • Comfortable working independently in the community and as part of a roster-based team.
  • Solid written communication for case notes, incident reports, and progress documentation.
  • Awareness of cultural diversity and the ability to deliver supports inclusively across CALD, First Nations, and LGBTIQ+ participants.

What You'll Need

  • Current Australian driver licence and a reliable, comprehensively insured vehicle (kilometre allowance paid in line with the SCHADS Award).
  • NDIS Worker Screening Check
  • Working with Children Check
  • National Police Check
  • NDIS Worker Orientation Module
  • Current First Aid and CPR certificate.
  • Right to work in Australia.
  • Availability to work rostered shifts across a 7-day roster, including some evenings and weekends.
