About the role

Mavin Living Pty Ltd is seeking an experienced and dedicated Practice Team Leader – Accommodation Services (Supported Independent Living) to join our growing team in SouthEast Melbourne (Cranbourne, CLyde Nth, Officer, Frankston). This is a full-time position that plays a crucial leadership role in delivering high-quality supported accommodation services to individuals with disability.

As Practice Team Leader, you will oversee the day-to-day operations of our Supported Independent Living (SIL) services, ensuring participants receive person-centred support that enhances their independence, wellbeing and quality of life. This role is essential to maintaining our commitment to delivering compassionate, responsive and effective disability support services that make a genuine difference in the lives of the people we serve.

This position provides two days per week administration and three days on shift with staff, so you can lead the way and mentor. Your shifts will be with staff, so this excludes shifts where nyou would be rostered on your own i.e. active night.

Key responsibilities

  1. Lead and supervise a team of support workers, providing guidance, coaching and performance management to ensure high standards of service delivery
  2. Develop and implement individualised support plans in collaboration with participants, ensuring services are tailored to each person's goals, preferences and assessed needs
  3. Oversee the day-to-day coordination of SIL accommodation services, including rostering, scheduling and resource allocation
  4. Monitor service quality and participant outcomes, identifying areas for improvement and implementing corrective actions as needed
  5. Maintain accurate and timely documentation, including case notes, incident reports and compliance records in accordance with NDIS standards and organisational policies
  6. Facilitate effective communication between participants, families, carers, external service providers and internal teams
  7. Identify training and development needs for team members and coordinate professional development opportunities
  8. Ensure compliance with all relevant legislation, regulations, and duty of care obligations, including safeguarding and risk management protocols
  9. Support participants in accessing community opportunities, building social connections and pursuing their aspirations
  10. Contribute to continuous improvement initiatives and quality assurance processes across the SIL service

Participate in a rotating After Hours On-Call roster.

What we're looking for

  1. Formal qualification in disability support, community services, social work or a related field (Certificate IV in Disability Support or higher preferred)
  2. Minimum 3-5 years' experience working in disability support services, with at least 2 years in a leadership or supervisory capacity
  3. Demonstrated experience delivering Supported Independent Living or similar accommodation-based support services
  4. Strong understanding of person-centred approaches and recovery-oriented practice principles
  5. Knowledge of the National Disability Insurance Scheme (NDIS) and its requirements for service delivery and participant outcomes
  6. Excellent team leadership and people management skills with the ability to motivate and develop staff
  7. Strong communication and interpersonal skills, with the ability to build positive relationships with participants, families and team members
  8. Proficiency in documentation and record-keeping, including experience with electronic case management systems
  9. Sound understanding of safeguarding, duty of care, and risk management in disability support contexts
  10. Current First Aid and CPR certification, including mental health first aid (or willingness to obtain)
  11. Current Working with Children Check and National Police Clearance (or eligibility to obtain)
  12. Strong organisational, problem-solving and decision-making abilities
  13. Commitment to ethical practice, dignity, respect and inclusion of people with disability
