At Lives Lived Well, we help people impacted by drugs or alcohol as well as mental health. We get lives back on track by delivering a wide range of programs and services (including residential rehab) across 40 locations in Queensland and New South Wales.

About the Role

As an AOD Counsellor Case Manager you will deliver individual counselling, case management, and group programs for our Logan House program, at Chambers Flats.

In this position you will be guided by your Team Leader to provide high quality engagement, assessment, counselling and support services to clients, and their families, using evidence based practice principles. In addition, manage a caseload of clients, including in an outreach capacity, by providing support, service coordination, referral and advocacy to assist client in achieving their goals.

The role will also include:

• Organise and facilitate therapeutic groups and day programs aimed at AOD intervention and relapse prevention
• Work with clients to develop, implement and review individual treatment goals
• Ensure continuity of care by effectively communicating within the multidisciplinary team and across interfaces with other health care providers

The successful candidate will need:

• Tertiary qualifications in a related area health service
• Applied knowledge in contemporary evidence based AOD counselling practice, case management and intake and assessment.
• Highly developed communication and community engagement skills.
• Experience in delivering education programs and facilitation groups (desirable).
• Local area knowledge (desirable).
• Current Australian Drivers Licence
