About the Role
We’re seeking an experienced NDIS Level 2 & 3 Complex Support Coordinator to join our growing team. In this role, you’ll support participants with complex needs to navigate the NDIS, connect with quality services, and coordinate supports that promote independence and wellbeing.
You’ll work closely with participants, families, and service providers to manage barriers, implement NDIS plans, and deliver high-quality, person-centred outcomes.
Key Responsibilities
- Provide Level 2 and Level 3 Support Coordination to NDIS participants with complex needs.
- Develop, implement, and monitor support plans tailored to individual goals.
- Build strong relationships with participants, families, and multidisciplinary providers.
- Coordinate crisis responses and manage risks effectively.
- Maintain accurate documentation and NDIS compliance standards.
- Collaborate with internal and external stakeholders to ensure seamless service delivery.
About You
You’re a compassionate, organised, and proactive professional who thrives on helping others succeed.
You have a strong understanding of the NDIS framework and the unique challenges faced by participants with complex needs.
Essential Criteria:
- Experience providing Level 2 and 3 Support Coordination under the NDIS.
- Comprehensive knowledge of the NDIS and community services sector.
- Strong communication, problem-solving, and advocacy skills.
- Relevant tertiary qualification (e.g., Social Work, Psychology, Community Services, or equivalent).
- Current Working with Children Check, NDIS Worker Screening Check, and Driver’s Licence.
Desirable:
- Experience supporting participants with psychosocial or dual diagnoses.
- Ability to work autonomously and manage a varied caseload.
