About the Role

We are looking for an experienced Registered Nurse who strongly aligns to our values and is wanting to step into a new challenge and make this role their own. Your confidence to work independently, ability to create and implement processes and resilient attitude will be highly regarded.

This is a full time role that will be based within Canberra and surrounding regional areas. Travel will be required as this is a community based role, where you may be servicing multiple clients per day in the community.

Key Responsibilities

  • Provide high level of health care assessments, clinical knowledge and expertise
  • Provide assessments of ageing and disability clients’ health status
  • Develop and monitor health care plans around meeting the changing health needs of individuals
  • Provide training and support for carers regarding health care plans
  • Educate and assess personnel, including support workers and carers, on how to support clients with complex health needs
  • Ability to implement and adhere to process guidelines with internal and external stakeholders

Skills & Experience

  • Degree qualification in Registered Nursing and current registration with appropriate board
  • Significant experience working with people with a disability/ageing clients who have high support medical needs
  • Current knowledge and demonstrated experience of clinical practice in the area of health planning
  • Key skills required include; effective time management, organisational skills, strong verbal and written communication skills
  • Ability to maintain high degree of confidentiality
  • Current and unrestricted Drivers Licence

Successful candidates will be required to clear probity checks including National Criminal History Record Check and Working with Vulnerable People Check (where relevant to the role).
