The Continuing Care Worker will provide ongoing oversight, support and referrals to clients from the Enhanced Engagement Program living in the community. The role will use active engagement skills to develop rapport with clients whilst they are in crisis accommodation, with a view to having an established relationship once the client moves into the community. The worker will continue to support the clients once housed, to support them to build connections within the local community.

We are currently seeking a skilled & passionate individual, someone committed to working with people who have previously experienced homelessness, engaging with them to support their independence and their journey within stable permanent housing.
