Position Purpose:

As a Disability Support Worker for Journey Health Solutions, you will assist clients to work towards the identified goals in their support plan. Using Person Centred Active Supports and a Recovery-Oriented approach, you will support clients by:

  • Supporting clients to access the community and participate in activities such as education, social activities and others as identified by the client.
  • Providing assistance with personal care including showering, meal assistance, grooming and other tasks as required.
  • Assisting clients to build skills in the areas identified in their support plan and support them to develop independence and increase choice and control.
  • Assisting clients to access and purchase items with their own money.
  • Where required, implementing strategies described in a clients Behaviour Support Plan.
  • Develop and maintain positive communication links with other service providers and support to individual Participants.
  • Maintain positive and welcoming relationships with family, friends and other services providers of Journey Health Solutions.
  • Read and update Participants Progress Notes and Files as required and ensure relevant documentation is available to other Journey Health Solution staff.
  • Record attendance and complete time sheets on the mobile APP according to Journey Health Solutions policy.
  • Communicate verbally or in writing, any observations that may affect Participants activities and the running of the service.
  • Contribute to maintaining an effective team.
  • Attend and participate in meetings, reviews and committees as required.
  • Work in accordance with all Journey Health Solutions Policies and Procedures.

Essential Qualifications

  • Minimum 3 months experience in a Disability Support Worker role
  • Certificate III in Disability (essential) or,
  • Certificate IV in disability or equivalent or,
  • Certificate III in individual support

Desirable

  • Experience and further qualifications in the area of mental health/psychosocial disability

Other Requirements

  • Ability to work flexible hours
  • National Police Check
  • International Police Check or Statutory Declaration if you have resided overseas more than 12 months in last 10 years
  • Disability Worker Exclusion Scheme Check (Conducted by Journey Health Solutions)
  • Working With Children’s Check
  • Current driver’s licence and reliable transport
