The Clinical Care Partner is a specialist role within the Care Partnering team, responsible for advising/leading care management and coordination of complex and high-risk clients across all aged care funding streams including Support at Home, CHSP, Claims Conference and Private Services.

As a registered health professional — a Registered Nurse with current AHPRA registration, an AHPRA-registered allied health professional, or a social worker eligible for AASW membership — the Clinical Care Partner brings clinical expertise to the team, providing specialist input, shared clinical decision-making and escalation support to their Care Partner team members.

What You'll Do

This role operates as a trusted specialist peer and clinical reference point when clients deteriorate or are complex and high-risk to support at home. The Clinical Care Partner works closely with Care Partners on shared clients and independently manages their own caseload of clients that require ongoing clinical oversight. This includes but not limited to:

  • Carry a direct caseload inclusive of complex and high-risk clients, providing expert care management and clinical oversight
  • Assist with clients identified as complex or high-risk (e.g. multiple or unstable conditions, recent hospitalisation, cognitive impairment, high falls or medication risk, deterioration)
  • Lead or support care planning, clinical review and coordination for complex or deteriorating client presentations.
  • Serve as the primary clinical reference point for Care Partners when clients present with elevated risk, complexity or deterioration
  • Share clinical decision-making and risk escalation alongside Care Partners
  • Monitor client service use against the approved budget under Support at Home, CHSP and other programs

What We're Looking For

  • Current professional registration in one of: Registered Nurse (AHPRA); allied health professional (AHPRA, e.g. physiotherapy, occupational therapy); or social worker eligible for AASW membership via an AASW-accredited qualification
  • Understanding of care planning and aged care funding programs
  • Demonstrated skills in care coordination and communication
  • Postgraduate study or demonstrated experience in aged, community or complex care; case management experience
  • Knowledge of trauma informed and care practices and willingness to include Jewish care values into their everyday practice
