Inspired by the spirit of the Daughters of Charity, The House of Welcome seeks to further social justice by standing with disadvantaged people offering support, solutions and hope.  We have over 50 years’ experience providing services to people experiencing marginalisation, disadvantage, social isolation and homelessness and have a non-judgmental ‘open door’ service approach to our community. 

About the role

As part of a committed team who love what they do and who work hard to make a real and positive difference in the lives of others, you will provide support coordination to NDIS customers and assist them in achieving the goals set out in their NDIS plans. Key responsibilities include:

  • Working with clients on a 1:1 basis - engaging them to develop and refine their goals, helping build their capacity and independence, assisting them to implement plans, providing information and advice regarding available psychosocial supports and program options, coordinating their services and supports, submitting applications or re-testing eligibility and managing billing
  • Engaging and coordinating service providers on behalf of participants and linking participants to relevant providers
  • Engaging participants informal supports (family, carers, etc.) and providing crisis support
  • Building knowledge of local services and organisations customers can utilise
  • Engaging with NDIA staff and LAC’s

Key requirements

  • Relevant NDIS experience preferably within mental health and/or homelessness with knowledge and experience of NDIS access processes for psychosocial supports
  • Relevant tertiary qualifications (Certificate IV or above)
  • Fluency in written and spoken Vietnamese will be an advantage but is not essential
  • Excellent communication (verbal and written), negotiation and relationship management skills
  • Demonstrated experience in working with healthcare providers including mental health and AOD providers, broader community services, General Practices, nurses, and Allied Health Providers and clients
  • Proven ability to identify innovative solutions and influence outcomes
  • Clear and Current National Police Check, Working with Children’s Check and Disability Worker Exclusion Scheme (DWES) Check
  • A strong orientation and commitment to social inclusion and diversity with the ability to work with diverse groups including Aboriginal and Torres Strait Islander, refugee groups, clients with CALD backgrounds and LGBTIQ groups
  • Right to work in Australia and a commitment to working on a part-time basis as a preference
  • Commitment to the mission and values that underpin the work of The House of Welcome
