About the role

We are seeking an experienced and passionate Case Manager to support young people aged 16–25 who are experiencing or at risk of homelessness. Working within a youth-focused housing and support service, you will provide case management, practical housing assistance, tenancy sustainment support and group-based interventions that help young people build stability, independence and connection.

The Case Manager will work alongside young people to assess their housing and support needs, develop individualised support case plans, strengthen independent living skills, connect them to relevant services and support them to achieve sustainable housing outcomes.

As part of a collaborative team, you will contribute to service development, continuous improvement and the meaningful participation of young people in shaping programs and supports.

What’s YOU'LL BRING:

  • Ability to build genuine, trusting relationships with young people through a strengths-based and trauma-informed approach.
  • Strong written communication skills, including case notes, support plans, referrals and reporting requirements.
  • Ability to work collaboratively with internal and external stakeholders
  • Proficiency in Microsoft Office Suite, with experience using Canva desirable (not essential)
  • Current Queensland Driver Licence
  • Current Blue Card
