The opportunity

Join a collaborative and purpose-driven Health Promotion team dedicated to improving the health and wellbeing of communities across Melbourne's Inner East and Yarra Ranges. As a Health Promotion Practitioner, you will lead and support evidence-based health promotion initiatives across community, education, sport and recreation settings, working alongside local government, community organisations and key stakeholders to deliver population health outcomes. This is an opportunity to drive meaningful, place-based prevention strategies, influence system-wide change, and contribute to healthier communities through partnership, innovation and engagement.

Reporting to the Manager, Health Promotion, this Permanent Fulltime role is based in Hawthorn, offering the flexibility and the opportunity to work across Access Health and Community sites of Lilydale and Belgrave as required. What you will be doing

• Lead the planning, delivery and evaluation of health promotion programs and population health initiatives aligned to Community Health – Health Promotion (CHHP) priorities and annual action plans
• Build and maintain strong partnerships with community, community organisations, local government, education and sport and recreation settings to drive sustainable health and wellbeing outcomes
• Develop high-quality project documentation, reports, communications, advocacy materials and grant applications to support successful program delivery and funding outcomes
• Facilitate stakeholder engagement, community participation and cross-sector collaboration to strengthen prevention and health promotion initiatives across diverse communities
• Monitor, evaluate and report on program outcomes while identifying new opportunities, partnerships and evidence-based approaches to improve population health and wellbeing.

Key Selection Criteria

What you will bring:

• Tertiary qualifications in health promotion, public health, health science, community development or a related discipline
• Minimum of one years’ experience in a population health role within a community context
• Strong stakeholder engagement and relationship-building skills, with the ability to work collaboratively across teams and community partners
• Excellent written and verbal communication skills, including experience preparing reports, project documentation and funding submissions
• A proactive, community-focused approach with a passion for health equity, prevention and creating systems-level change that leads to positive community impact.

Compliance Requirements:

• National Police Check, Working with Children Check
• Evidence of the right to work in Australia and a valid Driver’s Licence.
