Are you an experienced and passionate Accredited Practising Dietitian (APD) looking to make a real difference in community health? Join Access Health and Community and play a pivotal role in delivering exceptional, client-centred nutrition services that improve health outcomes and empower individuals to live healthier lives.
As a Grade 2 Dietitian, you will be part of a highly regarded multidisciplinary Allied Health team, providing evidence-based nutrition assessment, medical nutrition therapy, and education to adult and paediatric clients across a broad range of clinical conditions and funding programs.
Working within a diverse and rewarding community health setting, you will support clients funded through the Commonwealth Home Support Programme (CHSP), HACC-PYP, Community Health, Support at Home, Medicare, National Disability Insurance Scheme (NDIS) and Private Fee-for-Service programs. You will collaborate closely with clients, families, carers, support workers, and healthcare professionals to develop personalised nutrition plans that promote independence, enhance wellbeing, and improve quality of life.

As a Grade 2 Dietitian, you will provide comprehensive, person-centred nutrition services to both adult and paediatric clients through a range of service delivery models, including:

Working as part of a multidisciplinary Allied Health team, you will contribute to holistic care planning and provide tailored, evidence-based nutrition strategies that support positive and sustainable health outcomes.

Reporting to the Allied Health Manager Yarra and Boroondara, this is a Maximum-term part-time position (0.6 EFT) until July 2027. This position is primarily based at our Hawthorn site, with one day a week at Lilydale and within the community. The successful candidate must be able to work a Wednesday, and the other days are negotiable.
