Hope Street Youth and Family Services Ltd ("Hope Street") has over 35 years of experience delivering specialist services to young people and their families experiencing, or at risk of, homelessness.
Hope Street First Response Youth Support Service in Melton is a new innovative program funded by the State Government in response to youth homelessness in the growth corridor of the City of Melton.

We are looking for a positive and dedicated Case Worker for a part-time position working a 9-day fortnight in Melton. In this role you will provide short term intensive case management services to a young person and their children responding to their immediate (emergency) and short term needs within a holistic client focused approach; including: housing, health, specialist health, employment, education and training, family/significant relationships, legal, community linkages and/or participation. You will proactively engage, motivate and support the young person to develop interdependent, sustainable life options with a focus on achieving their case plan goals. Your relationships with key support services will enhance young people’s connections to those services and you will maintain accurate client records compliant with DHHS requirements.
The roster is Monday to Saturday (68.4 hrs fortnight) including day and evening shifts.
Classification: SCHCADS Award 2010, Level 4

You have a Bachelor Degree qualification in Social Work, Youth Work, Family Therapy, Psychology or other health/welfare related area. You are a motivated case manager with 3 years experience ideally working with youth. You have an understanding of the youth homelessness service system and allied services best placed to support and divert young people from homelessness. You have great relationship building skills which enables you to motivate young clients in crisis settings to achieve meaningful goals within a case management framework. Your friendly and cooperative nature means that you can build relationships quickly with key stakeholders in the community. You have highly developed communication skills including report writing, listening, negotiation, problem solving, decision making and advocacy. You are highly competent using Microsoft applications and Case Management Software.
