Home Caring is a reputable and fast-growing provider for Aged Care services, nationwide. We are currently seeking a passionate and experienced Case Manager to join our team and support a growing number of clients in the Narre Warren area.

This role is ideal for someone who is community-minded and committed to providing high-quality, person-centred care. This is a full-time role that is working in the office and in the community.

Key Responsibilities

• Conducting client assessments and developing person-centred care/support plans
• Coordinating services, monitoring outcomes, and reviewing care plans regularly
• Advocating for clients and ensuring their cultural, linguistic, and health needs are met
• Supporting clients to access appropriate services and resources
• Liaising with families, support workers, and health professionals
• Ensuring compliance with the Aged Care quality standards
• Enrolling and management of Home Care Packages for Aged Care clients

What We’re Looking For

• Relevant HCP & SAH Case Management experience
• Experience in Aged care or community health settings
• Strong communication, problem-solving, and organisational skills
• Empathy, cultural awareness, and a passion for helping others
• Current First Aid & CPR, and NDIS Worker Screening check
• Driver's Licence and reliable vehicle
• Full working rights in Australia
• Ability to assist the growth of the business by sourcing and signing up clients
