As the Coordinator of this innovative program, you will:

  • Support uptake of viral hepatitis testing and treatment by proactively engaging with people who can benefit from connection to care to offer information, education and service navigation and referral
  • Focus on network development ensuring clear health pathways, and increased engagement with other funded programs to support people in need by providing linkage to care in their local communities
  • Coordinate person centred care through facilitating referrals and strengthening pathways
  • Maintain existing, and establish, new networks that reduce the impacts of stigma and discrimination for people seeking treatment or support
  • Undertake proactive engagement across NSW to identify potential linkages, facilitate partnerships and support new program establishment opportunities
  • Support and assist promotion and marketing of the program, and
  • Collect and record data in relation to the program.

THE ROLE

We are looking for an enthusiastic person to grow and promote our HepLink NSW Program.

This role provides person‑centred support to individuals and services engaging with HepLink NSW, delivering guidance, referrals, and outreach while maintaining strong local networks that enhance access to viral hepatitis care. It also collaborates with national teams, collects program data, and drives awareness to ensure seamless, effective implementation of HepLink across NSW.

You will be part of the Community Support and Engagement team developing and delivering innovative and practical people focused programs to support the elimination of viral hepatitis in NSW by 2028.

HEPLINK NSW

The HepLink NSW project contributes to the national HepLink Australia program. This program reaches out to people with important information about the need for hepatitis B and hepatitis C testing, vaccination, monitoring or treatment. HepLink NSW provides linkage to care through state-based, readily accessible, up-to-date information and contact details of local health service providers who deliver hep B, hep C and liver health care. The role also provides information and support over the phone, face to face in the community, via email and instant chat.

We are looking for someone who:

  1. Has the drive to support the health and wellbeing of people living with, or at risk of, viral hepatitis.
  2. Has a passion and ability to build and develop partnerships that support HepLink uptake and improve statewide pathways for hepatitis care.
  3. Can build, maintain, and promote the Hepatitis NSW Service Directory and facilitate service connections, referrals, and access to relevant local supports.
  4. Can engage communities, connect services, support workforce training, and provide information to enhance access to, and seamless delivery of, HepLink NSW.

WORKING AT HEPATITIS NSW

Our vision is for a world free of viral hepatitis. We provide information, support, referral, and advocacy for people affected by viral hepatitis in NSW. We work to prevent the transmission of hepatitis B and hepatitis C and to improve the health and well-being of affected people and communities.

  • Location: Hepatitis NSW (Surry Hills) Office.
  • Salary: $51.00 to $53.31/hour (plus superannuation and leave loading) – commensurate with skills, qualifications, and experience.
  • Hours: Full-time, 5-days a week (70 hours per fortnight)

SELECTION CRITERIA

In your application, please respond to the following criteria, providing specific detailed examples that demonstrate you have the qualifications, knowledge, experience, skills, and abilities identified in the Position Description.

  1. Detail your approach, skills and experience in identifying, building and supporting the development of partnerships and networks.
  2. Based on your knowledge, describe an ideal experience of person-centred service delivery, from the consumer’s viewpoint.
  3. Provide a summary of your experience where you provided group work or adult education, information or training to service providers to increase their knowledge and support people’s access to services.
  4. Outline your experience in Microsoft Office suite of programs and Client Record Management (CRM) software you have used.
  5. Outline strategies you employ in working collaboratively across different teams.
