Role overview:

We currently have multiple exciting opportunities for Senior Mental Health Support Workers available. This role will be offered on a full time fixed term basis currently contracted until June 2020. You will be based from our Guildford head office with a view to move closer to the Perth CBD in the new year.

A Senior Mental Health Support Worker will assist clients with mental health challenges who are living within the community and who may have coexisting disabilities and/or complex needs. This will be obtained through the delivery of community based, person centred strategies. The role will focus on the goals contained within their Recovery Action Plan. 

The incumbent will be responsible for the assessment of clients to develop their Recovery Action Plans, in this you will work closely with clients, their families & carers towards capacity building in the areas of community access, inclusion and day to day living, with support from the Team Leader.

Sample of key responsibilities:

  • Conduct initial assessments of needs and develop a Recovery Action Plan
  • Provide one to one outreach support for people with severe mental illness towards capacity building in the areas of community access, inclusion and day to day living as identified in their Recovery Action Plan
  • Exercise a considerable degree of independent judgment in assessments, implementation and treatment planning with support from the Team Leader as required

Essential minimum selection criteria:

  • Qualification in the mental health, community disability, or welfare sector
  • Minimum 3 years’ experience working in a similar role providing mental health support work to clients in an outreach capacity
  • An understanding of mental health issues and recovery oriented practice in the provision of capacity building support to people with severe mental illnesses and complex needs and their families
  • High level of attention to detail with the ability to manage confidential information
  • Demonstrated ability in developing and interpreting care plans to support clients in achieving set goals
  • Excellent time management skills when working in time limited environments
  • Maintained knowledge of 360 Health + Community programs and relevant programs in other organisations
  • Strong communication (verbal and written) and negotiation skills
  • Demonstrated experience in maintaining accurate client information in data management systems
  • Experience in building and maintaining effective relationships with internal and external stakeholders
  • Experience in facilitating referral processes to internal and external stakeholder
  • Valid driver’s license  and access to own car for travel
  • A National Police Clearance dated within 6 months or willingness to obtain
