Gateway Health is a not-for-profit community health service delivering primary health and welfare services to individuals, families, and communities across Northeast Victoria and parts of Southern New South Wales. This is a rewarding part-time opportunity (60.8 hours per fortnight) based in Wodonga with the Pathways to Good Health Program, where you'll help improve the health and wellbeing of children and young people entering statutory care across the Ovens Murray region.
Working under the guidance of the Nurse Coordinator, you'll coordinate referrals, undertake health assessments and work alongside a multidisciplinary team to ensure children receive timely, holistic care. You'll build strong relationships with Child Protection, residential care providers and other key stakeholders, helping develop health management plans that support better long-term outcomes.
This role is suitable for applicants with nursing, allied health or broader health services background who enjoys coordinating care, building strong relationships and working collaboratively to improve outcomes for children and young people.
