Mental Recovery Planning Coordinator - Geraldton

This role oversees the clinical and casework aspects of the recovery program to ensure that we offer all residents a high level of case management and recovery support. This role will advise and oversee resident care and provide expertise in caring for residents with severe and persistent mental ill health from a clinical perspective. We are looking for someone who is Mental Health Nurse, Occupational therapist, or Mental Health Social Worker and has a Bachelor's or higher degree and 5-10 years of experience in a community, hospital or other clinical mental health setting. For the right candidate we will discuss relocation costs.

Key Responsibilities

  • Oversite of case management and clinical services.
  • Oversite of Medication Management
  • People and Culture - creating psychological safety for team
  • Leading and supervising Casework Team.
  • Working with other specialist services eg Mid West Mental Health
  • Ensuring high level case management and clinical support of Residents.
  • Final approval of residents entering and being exited from the program.
  • Supervision of case management team.
  • Representation of Fusion in Mental Health Sector.

This person will have experience working in Mental Health Service Delivery either in the community or Clinical setting and be able to lead a Case Management Team to provide a High Level of Recovery Support and Case Management. Understanding of presentation of Mental Ill Health and experience monitoring for decline and to do routine assessments, Experience providing clinical and practical supervision.

Key Tasks

  • Monitoring Medication Management
  • Delivery of PD for team
  • Resident intake and selection
  • Ensuring best practice and regular
  • Providing guidance on how to work with residents with challenging behaviour.
  • Developing Recovery Casework
  • Supervision of caseworkers and critical incident debriefing for staff.
  • Ensuring Communication between FMHU and MWMH
  • Overseeing all documentation regarding residents
  • Participating as part of on-call roster
    Approving leave and timesheets of staff in the team.
  • Working on Exit plans for resident
