The position

The Cradle to Kinder Practitioner position is a Full Time, Fixed Term role based between our Frankston and Sandringham sites. The position is responsible for providing intensive and therapeutic long term case management to promote the wellbeing of infants, children and vulnerable families. The aim of this program is to implement early intervention to young families who are experiencing significant vulnerabilities.

The Cradle to Kinder Practitioner provides long term case management to young parents and their infant, whilst employing a whole of family, holistic approach. Practitioners will support families based on their needs and will work closely with other key agencies as part of a care team approach, working particularly closely with DHHS Child Protection and Maternal and Child Health. A typical day in the life of this position includes conducting home visits to families, supporting parents and families to link in with their community, supporting families with a range of needs, facilitating groups, undertaking case noting and participating in peer supervision. During COVID we still maintain a high level of contact with families using virtual platforms.
