We provide a wide range of disability and community-based services across Greater Sydney, including high-intensity supports, supported independent living (SIL), complex support coordination, and innovative community participation.

We are expanding our community engagement efforts and seek an experienced and passionate Community Worker to join our diverse, person-centred team. This role is crucial in connecting participants to their local community and enhancing their social and emotional wellbeing.

Position Summary

The Community Worker is responsible for supporting individuals - particularly people living with disability - to actively participate in community life and access support services. The role requires engagement with clients, families, community groups, service providers, and government agencies to promote inclusion, independence, and wellbeing.

Key Responsibilities

  • Identify individual and community needs through assessment, outreach, and consultation.
  • Develop, implement, and review individualised community participation plans in partnership with clients and their support network.
  • Facilitate access to community services, including recreational activities, social groups, educational programs, and volunteer opportunities.
  • Establish and maintain partnerships with local organisations to improve service integration and support networks.
  • Advocate for the rights and interests of individuals within the broader community and service systems.
  • Support clients in navigating government and non-government support services, including housing, employment, and health-related resources.
  • Monitor, evaluate, and document client progress in accordance with their NDIS goals and reporting requirements.
  • Maintain accurate records, case notes, and reports to meet organisational and legislative requirements.
  • Participate in service planning, quality improvement, and risk management processes.
  • Promote cultural safety, inclusivity, and trauma-informed practice at all times.

Essential Criteria

  • Diploma or higher qualification in Community Services, Social Work, Human Services, or a related field.
  • Minimum 2 years’ experience in a community-services or case management role (NDIS experience highly regarded).
  • Demonstrated understanding of the NDIS Quality and Safeguards Commission requirements and person-centred practice.
  • Strong cross-cultural communication and interpersonal skills with the ability to build trust and rapport with diverse client groups.
  • Excellent problem-solving, organisational, and time management skills.
  • Proficiency in using client management systems and Microsoft Office Suite.
  • Current Australian driver’s licence and access to a reliable vehicle.
  • Valid Working with Children Check and NDIS Worker Screening Check (or willingness to obtain).

Desirable Criteria

  • Experience working with people with psychosocial disability, CALD communities, or First Nations peoples.
  • Fluency in a second language is advantageous.
  • Experience facilitating group programs or community engagement events.
