The Family Support Worker – Priority Response is a new role role. The focus of the role is on providing prompt and effective psychosocial support and spiritual care to clients with specialist palliative care needs and their families where the client lives in a Residential Aged Care Facility (RACF) and to those clients accepted for priority assessment by EPC’s Priority Assessment Team (PAT). Priority Assessment must occur within four hours of acceptance of referral.

To be successful, you will have:

  • Tertiary Qualification within the field of Social Work, Psychology or Pastoral Care/Theology with two completed units of Clinical Pastoral Education
  • Eligibility for membership of, or registration by, relevant professional bodies
  • Experience in and knowledge of aged care, palliative care and/or community health setting
  • Strong clinical knowledge and skills
  • High level triage and assessment skills in complex and crisis situations
  • Knowledge of current loss, grief and bereavement theory and practice
  • Well-developed interpersonal, oral and written communication skills and presentation skills
  • Current Victorian driver’s license, good driving record and willingness to use own

Part time- 64 hours per fortnight (4 days per week with either Tues, Wed or Thurs off each week).
