About the role
This is an exciting opportunity for a motivated and self-driven NDIS Support Coordinator who is passionate about person-centered, strengths-based practice.
You will support participants to understand, implement and optimise their NDIS plans while building their capacity to make informed choices and achieve their individual goals. You will work closely with participants, families, providers and mainstream services to ensure supports are effective, coordinated and empowering.
Your key responsibilities will include
- Supporting participants to build capacity, independence and confidence in managing their supports
- Planning, developing and coordinating services aligned with participant goals
- Liaising with service providers to maximise outcomes and ensure quality supports
- Monitoring, reviewing and adjusting plans in collaboration with participants and their networks
- Building strong relationships with NDIS, community and mainstream services
- Providing clear guidance and support to stakeholders as required
- Maintaining accurate, timely and compliant records

We are currently seeking local professionals based in:
Newcastle / Hunter / Maitland | Central Coast | Mid North Coast (towards Port Macquarie)
Essential qualities, skills & experience
- Tertiary qualification and/or significant experience in Community Services
- (Disability, Mental Health, Housing, Justice or related fields)
- Strong working knowledge of the NDIS, relevant legislation and best-practice Support Coordination
- Commitment to participant choice, control and capacity building
- Excellent interpersonal skills and cultural awareness
- Proven ability to build and maintain strong professional relationships
- High-level written and verbal communication skills
- Confident using Microsoft Office and online systems
- Ability to work autonomously while contributing to a remote team
Requirements
- NDIS Worker Screening Check (or willingness to obtain)
- Current driver’s license and comprehensively insured vehicle
- ACN, Public Liability & Professional Indemnity Insurance
- Laptop, mobile phone and private home office space
