ECSC is seeking a Multicultural Social Support and Inclusion Worker to provide group and individual social support services to assist frail older people to participate in community life and feel socially included through structured, group-based activities or individual services that develop, maintain, or support independent living and social interaction whilst facilitating their wellness and reablement goals.

You will work with our participants in Eastern Sydney and be based at our Marrickville office. The clients speak Greek, so fluency in the language is required.

KEY RESPONSIBILITIES

Key Responsibilities include but not limited to:

Group Social support:

• Organise and deliver group-based activities held in a facility/centre e.g. interest-based and individually tailored activities promoting physical activity, cognitive stimulation, and emotional well-being.
• Group excursions.
• Online group activities

Individual Social support:

• Assist an individual, either within the home environment or while accessing community services, e.g.
• Visiting services
• Telephone and web-based monitoring services
• Accompanied activities (such as assisting the person through accompanied shopping, bill-paying, attendance at appointments, and other related activities).
• Regularly assess and reassess the changing needs of the clients, establishing client-centred goals
• Maintain progress notes and report the hours provided to MACS clients in the Data Exchange

SKILLS & EXPERIENCE

Selection criteria:

• In-depth understanding of the cultural context of the community requiring support, including challenges in accessing My Aged Care and Commonwealth Home Support Program.
• Knowledge and understanding of community development values and principles.
• Ability to build relationships and maintain trust with program participants, organisations, and other stakeholders.
• Proficiency in written and oral language skills of the identified community requiring support.
• Experience providing social support services to seniors from CALD backgrounds based on a wellness and enablement approach.
• Experience organising and delivering culturally and linguistically appropriate individual support services to seniors from CALD backgrounds based on a wellness and enablement approach.
• Knowledge and proficiency with IT including for reports/care plans/progress notes writing, data entry, etc
• Ability to work within a supervised reporting framework.

Desirable:

• A second language (Ability to speak, read and write)
• Experience in providing services to older people and their carers from CALD backgrounds.
• Current driver’s license and access to a comprehensively insured car
• Certificate IIII or above in Aged Care / Individual Support / Community or willingness to obtain.
