About the role

Homelessness is emerging as a significant issue across the Eastern region of Melbourne and it's time for a dedicated team and approach in the Adult Mental Health Service to work with these consumers in achieving positive recovery outcomes.

You will have demonstrated skills and values of:

• Agility and flexibility
• Proven clinical and operational Leadership
• Compassionate with strong skills in engagement and rapport building
• Recovery oriented practitioner seeking to promote consumer choice and control
• Strong advocate for a shared care approach with all supports around the consumer
• Excellent mental state and risk assessment skills - with equally well developed risk mitigation and management skills promoting dignity of risk
• Great team player

About you

To be successful in this role you should possess:

• Case management experience and skills
• Leadership experience
• Current registration with AHPRA or equivalent for SW
• Highly developed interpersonal and leadership skills
• A strong commitment to high quality care
• The ability to work as part of a diverse team
• Excellent communication and organisational skills
