About the Role

An opportunity exists with the Chandler Continuing Care Team for a passionate and enthusiastic Occupational Therapist or Social Worker to develop competency in community mental health practice including discipline specific and case management responsibilities. The position will work in partnership with consumers, carers, families and other service providers in order to provide high quality discipline specific and mental health assessment and consultation services within a collaborative recovery oriented framework.

About You

• Qualified Mental Health Professional - OT or SW
• Mimimum of 12 months Mental Health Experience
• Well-developed interpersonal skills
• Ability to work as part of a diverse team
• Strong commitment to high quality recovery based case management
