About the role

We're looking for a committed Case Manager who wants to help clients experiencing homelessness transition into stable accommodation.

The team is based out of our Wantirna South, Knox Ozone site as a part of EACH's new Homelessness to a Home (H2H) program. The H2H Wellbeing Case Coordinator role is needed full-time (38 hours per week) until 31 August 2022.

What you will be doing

The Wellbeing Case Coordinator will support clients with their social and emotional wellbeing, helping clients build resilience and recovery in the areas of mental health, substance use, physical health, and life skills so that they can confidently transition to living in stable accommodation.

In this role, you will:

  • Use a strength-based approach to the role, looking for opportunities to complement and support existing client strengths and capacities.
  • Work with the client to co-create individual support plans, establishing and maintaining strong, supportive, and professional working relationships with clients.
  • Monitor and respond appropriately to significant homelessness risk, and risks that threaten housing stability.
  • Ensure clients are referred to appropriate supports, engaging other stakeholders as needed to improve clients' physical and mental health and wellbeing.
  • Encourage and support clients to access and remain engaged with their current support networks.
  • Provide ongoing support and connections/referrals to expand support networks.

Who we're looking for

You'll have:

  • A self-directed passion and drive to support people experiencing homelessness.
  • A demonstrated understanding of the context and systems associated with housing and homelessness.
  • Strong risk management experience, particularly with vulnerable people who may have complex needs.
  • Connections to resources and supports in communities spanning Melbourne's Eastern suburbs.

A tertiary qualification in Human Services, Social Work, or other relevant discipline/profession is highly regarded.
