Job Description

Join EACH as a Service Coordinator– STRC (Short-Term Restorative Care) and play a vital role in coordinating services and supports for older adults with Individual Support Packages.

In this position, you will work collaboratively with clinical case managers to ensure clients receive the appropriate support outlined in their plans. You will be responsible for liaising with various stakeholders, including government and non-government service providers, to advocate for the needs of the individuals supported by EACH. Additionally, you will oversee service access and intake processes for new referrals, and manage financial requirements in alignment with clients' Individual Support Plans/packages.

About you

To excel in this position, you will have the following qualifications, skills and experience:

  • Diploma in Health (or equivalent), or demonstrated industry experience
  • Significant experience in planning and implementing community-based services for older adults
  • A proven track record of maintaining accurate client records and written reports
  • A current state-based drivers license
