We are proud to partner with several Primary Health Networks (PHNs) to provide free psychiatric care to those most vulnerable and in need across each region. Our PHN-funded programs provide access to specialist mental health support for patients who would otherwise be unable to afford or access it - including people experiencing financial hardship, homelessness, or social disadvantage.

In addition to our PHN programs, we also have contracts and partnerships with several other programs. A growing part of our business is our partnership with insurers, where we deliver early intervention and coordinated care to help claimants recover faster, return to work sooner, and achieve better overall outcomes.

Role Summary

As a Clinical Care Coordinator, you will support patients throughout their journey in our funded programs, managing a variety of administrative and patient-facing tasks to ensure a seamless experience and compliance with contractual obligations. You will play a key role in appointment management, patient communication, accurate documentation, and high data quality - liaising with psychiatrists, referrers, and support teams, and contributing to Dokotela's commitment to high-quality care.

This role is based on-site at our Bondi Junction office.

Key Responsibilities

Referral and Intake Management

Triage incoming referrals, ensuring compliance with each program's eligibility criteria and suitability requirements. Provide referrers with timely updates on the triage process and outcome.

Accepted referrals progress to intake, which you will conduct via a call with the patient. During this call, you will ascertain the patient's needs and match them with the specialist best suited to their presentation. You will also identify any potential barriers to access and work to find practical solutions that support attendance.

Psychiatrist Liaison

Liaise with psychiatrists, referrers, and other relevant parties involved in a patient's care. This includes coordinating case conferences, chasing clinical documentation and past reports, and ensuring all parties are appropriately informed throughout the patient's journey.

Follow-up and Post-Appointment Management

Following up with patients after each appointment and monitoring their progress is a critical part of this role. You will support patients to engage with and implement their treatment plans, collect outcome measures, and maintain accurate data to enable comparison of outcomes and assessment of treatment efficacy.

Administrative and Compliance

Monitor and respond to several shared inboxes and communication platforms, managing correspondence with both patients and clinicians in a timely and professional manner. Beyond day-to-day administration, you will uphold our contractual obligations to PHN partners and other third parties, including accurate data management and reporting.

About You

  • Excellent organisational and time management skills
  • High attention to detail and commitment to accuracy
  • Clear and professional communication, both written and verbal
  • Empathetic and patient-focused approach to all interactions
  • Ability to multitask and self-direct in a fast-paced environment
  • Comfortable using practice management and clinical software systems
  • Tertiary qualification in Psychology, Social Work, Nursing, or a related clinical discipline (mandatory)
  • Minimum of 6 months' experience in healthcare administration (desirable)
  • Year 12 completion or equivalent (required where no tertiary qualification held)
