Deadly Connections is an Aboriginal community led not for profit organisation. As an Aboriginal controlled organisation, there is an inherent requirement that non-Aboriginal employees continue to develop their cultural competence to maximise opportunities and improve outcomes for First Nations peoples, families, and communities.

POSITION OBJECTIVES

Within a team approach and under supervision of the Team Leader/CEO, the Wellbeing Facilitator will:

• Provide grassroots, well-being support, case management, information, referral and advocacy to Aboriginal men who are participating in our program.
• Work within the decolonising model and abide by relevant policy practice guidelines.
• You will be expected to work within a collaborative team of stakeholders, including DCJ, health services, other Aboriginal and non-Aboriginal agencies to promote best outcomes for participants, their families and communities.
• Ensure the delivery of effective, high-quality services and outcomes to facilitate participants behaviour change through a multi-faceted program that provides individually tailored case management, information, referral, practical support, counselling, cultural/healing activities, education/employment support and cognitive behaviour therapy.

KEY RESPONSIBILITIES

• Create and maintain safety for women and children
• Work collaboratively with participants, families, and communities
• Program development and facilitation
• Provide individualised and tailored case management support in line with the Breaking the Cycle program and its specific needs.
• Develop care plans according to clients’ needs utilising strengths-based and decolonising practice.
• Work with internal and external staff & other Aboriginal services for all Aboriginal families regarding cultural connection and identity
• To work towards strengthening family/community connection and provide early intervention.
• To actively participate in teamwork in a manner that ensures a coordinated approach to services for participants and their families.
• Working across other support services programs as required providing support at times of crisis and when required.
• To undertake the responsibilities associated with intake and response, such as:
  • Receive, facilitate, and process referrals from stakeholders to the Breaking the Cycle program and to our partner agencies.
  • Making referral/s, completing timely and relevant follow up.
  • Coordinating an appropriate response for program participants
  • Entering data into the electronic system and creating paper files
  • Ongoing communication with key stakeholders
  • To work collaboratively with both internal and external partners and stakeholders
  • Liaise with internal and external programs in relation to capacity, referrals and other program information.
