The Role

The role of the NDIS Team Leader and Coordinator is to manage the day to day operation of Creating Links NDIS portfolio to achieve high standards of practice and service delivery through effective team management. This will include managing staff rosters, workflow management and ensuring staff meet and exceed our agreed contractual obligations and key performance indicators.

Additionally, as an NDIS Coordinator the role is expected to promote, develop, and expand the programs and participate in project work as directed.

• Engaging with NDIS participants to identify and refine their goals in line with their NDIS Plan.
• Provide pre and post NDIS planning support.
• Liaison with stakeholders.
• Attending meetings with participants, families, and nominees to facilitate services and supports to achieve NDIS goals.
• Providing and maintaining accurate and comprehensive reports in line with NDIA and creating Links policies and procedures.
• To research, coordinate and manage a range of supports to suit individual needs across multiple providers to meet NDIS goals.
• To support individuals to connect with their communities and encourage participation in both mainstream and community options.

Essential Criteria:

• Bachelor’s Degree in Habilitation / Social Welfare / Education / or a closely related field.
• Demonstrated experience and a high level of understanding of NDIS funding and applications.
• Significant team leading and staff coordination experience.
• Up to date knowledge of the policy, funding, and reporting environment for a not-for -profit organisation in the disability services sector.
• Excellent communication skills both verbal and written.
• Passionate about service excellence.
• Drivers Licence.
• Valid Working with Children Check and National Police Criminal History Check.

• As a not for profit entity the position includes a generous salary packaging option of up to $15,900 annually as well as reimbursement for reasonably incurred expenses.
