The Service Coordinator is the primary point of contact for all our clients whilst managing a team of support workers, who deliver our services to our participants living with various disabilities. This role is in Bundoora and training will be provided.

All responsibilities must be undertaken in line with the Policies and Procedures of Complete Nursing & Home Care Pty Ltd and under the NDIS guidelines. The Junior Service Coordinator embodies the company values of Respect, Integrity, Quality and Commitment.

  • Reports to: Operations Manager

POSITION REQUIREMENTS

Key Duties and Responsibilities

  • To work with individuals and their families to schedule care and support including personal care, clinical support and engaging with customer communities
  • To manage consumer directed care queries flexible and tailored coordination to meet specific customers’ needs and incorporate shared care planning
  • To use strengths-based approach together with a strong collaboration approach to assessing and matching carers and services in the areas of need where the customer and their immediate supports cannot provide
  • To demonstrate awareness of diversity and be respectful to all stakeholders of the enterprise
  • To demonstrate a commitment to ongoing professional development
  • To demonstrate effective communication skills with customers, staff, and allied health professionals
  • To demonstrate good time management and problem-solving skills
  • To understand the risks associated with the work and ways to mitigate these
  • To ensure services are delivered in line with company policies and procedures
  • To contribute to weekly management meeting agenda and attend meetings
  • To contribute to Complete Nursing newsletter and useful items
  • To contribute to the development of Complete Nursing &Home Care training
  • Other related task consistent with your skills and training

Qualifications

  • Relevant qualification in the disability sector (Desirable)
  • Knowledge of NDIS and Practice Standards
  • Must have a passion for working with Vulnerable people
