About the role

The Commonwealth Psychosocial Support (CPS) program provides time limited home and community-based supports to consumers with severe mental illness. This role will be responsible for maintaining an active caseload of up to 15 consumers at any point in time and may be required to support the development and implementation of group activities.

Your duties include:

  • Maintain an active case load, providing flexible one on one outreach support to consumers in their homes, community or via a modality that best meets their needs
  • Plan and facilitate day-to-day activities of the role and participate in program activities to ensure the provision of a high-quality service
  • Contribute to the development and implementation of local work instructions, processes, and procedures
  • Undertake discrete initiatives that increases the efficiencies and effectiveness of the program

Our ideal candidate:

  • Tertiary qualifications with demonstrated skills and minimum of two years' experience working in community mental health or related field
  • Excellent verbal and written communication, analytical thinking and inquiry skills
  • Proven ability to work in a complex and changing internal and external environments
  • Sound understanding of models of care, psychosocial support and best practice evidence-based interventions, including strengths-based and trauma-informed frameworks and the Collaborative Recovery Model.

All appointments are made subject to a satisfactory Police Check, a Working With Children Check (WWCC), complete a Statutory Declaration, evidence of legal rights to work in Australia, and evidence of vaccination against COVID-19 (3 doses). Additionally, some roles will also require current AHPRA registration and/or NDIS Worker Screening Clearance.

Roles that are determined to be Risk Category A or B [staff that have direct and/or indirect contact with body substances and are at higher risk of occupational exposure to infections] will be required to provide evidence of immunisation assessment including Influenza prior to commencement [A] or within 12 weeks of commencement [B]. This is in accordance with the Victorian Minister of Health Mandatory Vaccination Orders for COVID-19 and legislated requirement for influenza vaccination.
