The Team Leader Access and Navigation plays an integral role in facilitating opportunities and referral pathways with a range of stakeholders to improve access and equity in service delivery. Leading a team of Social Prescribing Link Workers, Case Managers, and a Crisis Support Officer you will support care coordination activities with the team as well as assist with developing intervention pathways for vulnerable clients.
