A bit about the role

The Team Leader Access and Navigation plays an integral role in facilitating opportunities and referral pathways with a range of stakeholders to improve access and equity in service delivery. Leading a team of Social Prescribing Link Workers, Case Managers, and a Crisis Support Officer you will support care coordination activities with the team as well as assist with developing intervention pathways for vulnerable clients.

What you’ll deliver

  • Develop communication content to promote programs and services as well as participate in regular networking events to increase awareness.
  • Engage and partner with health and community services sector to establish effective working relationships to enhance the sustainability of the Social Prescribing program.
  • Monitor and report on program performance outcomes and financial objectives to ensure budgeting and funding obligations are met.
  • Support and lead the team with a shared leadership approach which encourages collaboration and service excellence.
  • Develop policy framework, programs and activities that promote the need to live and age well within the community.

What you’ll bring

  • Excellent interpersonal skills and the ability to communicate with a diverse range of stakeholders and clients.
  • Strong project and time management skills.
  • Open-minded and inclusive leadership approach.
  • Highly developed analytical, negotiation and problem solving skills.
  • Tertiary qualification in Social Sciences or substantial experience in social policy, human services, allied health or within the community services sector.
  • Willingness to undergo pre-employment background checks including National Criminal History Check, Working with Children Check, Eligibility to Work check, Functional Assessment
  • A curious, collaborative, and inclusive community mindset.
