At Central Coast Local Health District (CCLHD), we are committed to growing and supporting our Aboriginal health workforce and delivering trusted care and better health for Aboriginal people on the Central Coast, and we are looking for a dedicated Mental Health Clinician to join our Care Coordination Team.

About the Opportunity

As a Mental Health Clinician within the Care Coordination Team, you will play a key role in delivering high-quality, community-based mental health care to adults experiencing mental illness and mental health concerns. Working as part of a multidisciplinary team, you will provide assessment, care coordination, support, and advocacy to help clients achieve positive and sustainable mental health outcomes.

This position plays an important role in supporting recovery-focused care and enhancing the wellbeing of individuals, families, and communities. In this role you will:

  • Conduct comprehensive mental health assessments and develop collaborative, person-centred care plans and recovery goals.
  • Manage an allocated caseload, providing ongoing care coordination, support, and monitoring for consumers receiving voluntary and involuntary treatment.
  • Work collaboratively with clients, families, carers, general practitioners, and community service providers to promote holistic care and positive outcomes.
  • Support clients to navigate health and social service systems, facilitating access to specialist services, housing, community supports, and other relevant resources.
  • Provide culturally appropriate mental health care, advocacy, education, and support, ensuring services are responsive to the needs of Aboriginal consumers and their communities.
  • Maintain accurate clinical documentation and contribute to quality improvement initiatives that enhance service delivery and consumer outcomes.

About You

We are looking for someone who has:

  • A Bachelor Degree in Occupational Therapy, Counselling, Community Welfare, Social Work, or Nursing, or a qualification as an Aboriginal Health Practitioner, with relevant professional registration where applicable.
  • Demonstrated experience delivering effective mental health assessment, treatment, support, and care coordination within community and/or mental health settings.
  • Knowledge and understanding of Aboriginal culture and the social and emotional wellbeing issues experienced by Aboriginal people, with the ability to provide culturally appropriate care and advocacy.
  • Strong communication, interpersonal, and relationship-building skills, with experience working collaboratively across multidisciplinary teams and external agencies.
  • A commitment to recovery-oriented, person-centred care and promoting positive mental health outcomes for consumers, families, and carers.
  • A current NSW Driver Licence.
