Ready to make a real impact in Social Support and community health? Better Health Network (BHN) is seeking a dynamic Team Leader, Social Services to lead a high-performing team from Mentone and other Bayside sites. This is an opportunity to drive service excellence, lead innovative community-focused programs, and improve wellbeing and outcomes alongside a highly motivated team.
Our Social Support program is available to people over age 65 (over 50 for Aboriginal or Torres Strait Islander People) who are eligible for My Aged Care or HACC funding. We provide small group outings, as well as centre-based gentle exercise programs, and creative experiences, in culturally sensitive manner, to improve connection, independence and wellbeing for older adults.
As Team Leader, Social Services, you’ll provide operational leadership across our Wellness & Support program, ensuring timely and high-quality service delivery that meets community needs.
