Better Health Network (BHN) is seeking a passionate and experienced health professional to join our Indigenous Access Team as an Care Coordinator.
This is an opportunity to work alongside Aboriginal and Torres Strait Islander individuals, families and communities, supporting people living with chronic health conditions to access the care, services and resources they need to thrive.
Working within a culturally safe and community-centred framework, you will build trusted relationships, coordinate care, advocate for clients, and contribute to improved health and wellbeing outcomes across Melbourne's south-east.
As the Integrated Team Care Coordinator, you will support Aboriginal and Torres Strait Islander clients to navigate the health system, manage chronic conditions and access the services they need to achieve their health goals.
You are a compassionate and skilled health professional who values relationship-building, cultural respect and community-led approaches.
