Better Health Network (BHN) is seeking a passionate and experienced health professional to join our Indigenous Access Team as an Care Coordinator.

This is an opportunity to work alongside Aboriginal and Torres Strait Islander individuals, families and communities, supporting people living with chronic health conditions to access the care, services and resources they need to thrive.

Working within a culturally safe and community-centred framework, you will build trusted relationships, coordinate care, advocate for clients, and contribute to improved health and wellbeing outcomes across Melbourne's south-east.

About the role

As the Integrated Team Care Coordinator, you will support Aboriginal and Torres Strait Islander clients to navigate the health system, manage chronic conditions and access the services they need to achieve their health goals.

You will:

  • Provide culturally safe care coordination and case management
  • Develop individual care plans in partnership with client
  • Support self-determination and health self-management
  • Connect clients with health, housing, mental health, AOD, legal and community services
  • Build and maintain strong relationships with Aboriginal community organisations and stakeholders
  • Promote access to culturally appropriate health services
  • Advocate for improved outcomes and service accessibility for Aboriginal and Torres Strait Islander peoples

About you

You are a compassionate and skilled health professional who values relationship-building, cultural respect and community-led approaches.

You will bring:

  • Nursing qualification with current AHPRA registration
  • Experience working with Aboriginal and Torres Strait Islander peoples and communities
  • Understanding of the historical, social and cultural factors that influence health and wellbeing outcomes
  • Experience supporting people with chronic and complex health needs
  • Strong care coordination, case management and advocacy skills
  • Excellent communication and stakeholder engagement capabilities
  • Current driver's licence
