About the role

As an Occupational Therapist Grade 2, you'll support adults, primarily older people with chronic and complex health needs to remain safe, independent, and engaged within their homes and communities.

You'll provide evidence-based assessment, intervention, education, and care planning while collaborating closely with clients, carers, and a broad multidisciplinary team.​

Your work will include

  • Functional and home-based assessments
  • Recommendations for assistive technology and equipment
  • Home modification assessments and applications
  • Supporting activities of daily living and independence goals
  • Client and carer education using a self-management approach
  • Working across programs including My Aged Care, CHSP, HACC PYP and Support at Home
  • Collaborating with Physiotherapists, Podiatrists, Nurses, Allied Health Assistants and external service providers
  • Contributing to quality improvement and service innovation initiatives

About you

You're a compassionate and capable Occupational Therapist who enjoys working autonomously while being part of a collaborative team.

You'll bring:

  • Current AHPRA registration as an Occupational Therapist
  • Experience in community, aged care, disability, rehabilitation or related settings
  • Knowledge of home modifications, assistive technology and functional assessment
  • Understanding of My Aged Care, CHSP, HACC PYP, Support at Home and/or disability services
  • Strong communication, clinical reasoning and problem-solving skills
  • Commitment to person-centred, evidence-based practice
  • Current driver's licence and ability to travel across the BHN catchment
