Are you passionate about supporting older people to live independently at home? Join Western District Health Service as a Support at Home – Case Manager, where you’ll deliver high-quality, person-centred care management and make a meaningful impact in your community every day.

This is a role for someone who thrives on building relationships, problem-solving, and coordinating supports that enhance quality of life, dignity, and choice for older people.

What You'll Be Doing

Person-Centred Care: Deliver tailored care management that supports independence, dignity, and individual choice for older people living at home.

Care Planning & Coordination : Develop, implement, and review personalised care plans in collaboration with participants, families, and service providers.

Stakeholder Engagement: Work closely with clients, families, healthcare professionals, and external providers to ensure coordinated, seamless care.

Budget & Program Management: Manage individual program budgets, ensuring supports are delivered effectively within funding guidelines.

Monitoring & Review: Regularly assess client outcomes and adjust care plans to meet changing needs.

Documentation & Compliance: Maintain accurate records and ensure compliance with program requirements, standards, and reporting obligations.

Teamwork & Collaboration: Work independently and as part of a multidisciplinary team within a dynamic and supportive environment.

What You Bring

  • Experience in aged care case management
  • Demonstrated ability to manage budgets and funding programs
  • Strong communication and relationship-building skills
  • Ability to coordinate complex supports in a community setting
  • Excellent organisational and time management skills
  • Commitment to person-centred care and positive client outcomes
  • Ability to work both independently and within a multidisciplinary team
