In this important role, you will work with our clients leaving care and support their voices to be heard in their own transition planning. The support will begin in care and post care and span such areas as housing, health & wellbeing, education, employment, community and cultural connections.
As the Advanced Practitioner you will work closely with the Team Leader and team by applying your specialised knowledge, guidance, advice and recommendations through your experience and practice. You will assist the Team Leader to support the team in the delivery of a high quality, responsive and effective service within a strong and professional risk assessment framework.
Tertiary qualifications in Social Work, Welfare Studies or their equivalent, or other tertiary qualification appropriate to public welfare practice.
You are an enthusiastic and committed individual with demonstrated skills and experience in engaging with young people and working collaboratively to achieve positives outcomes. Your highly developed cultural awareness and sensitivity, partnered with excellent communication skills, will enable you to quickly build strong relationships with our clients and key stakeholders. You are not afraid of a challenge and constantly seek to grow and develop.
