About role
We are looking for an experienced, empathetic Community Worker to be the heart of our service delivery. This isn't just a job; it's a chance to champion independence, foster genuine well-being, and empower participants to thrive in their community. You will be the trusted, consistent figure who works side-by-side with participants, families, and key support providers to deliver holistic, coordinated, and goal-focused support that truly reflects the individual's personality and ambition.
Duties and tasks:
- Assessing participant strengths, needs, and goals through regular engagement to ensure support plans remain relevant.
- Developing, implementing, and continually reviewing personalised support plans that actively promote independence and quality of life.
- Facilitating community-based activities that build confidence, encourage social inclusion, and develop practical life skills, including assisting access to essential services such as health, education, employment, and recreational programs.
- Liaising with families, carers, support coordinators, and external service providers to ensure support elements are integrated and coordinated.
- Upholding standards of participant dignity, choice, privacy, and safety, and maintaining detailed, accurate case notes in accordance with regulatory and organisational requirements.
Experience and Skills:
- A minimum of two years of relevant professional experience in disability, community services, aged care, or welfare support.
- Demonstrated ability to assess needs, plan support, and deliver structured, community-based programs autonomously.
- Exceptional communication skills and proven ability to build and maintain trusted, supportive relationships.
- A highly organised, flexible, and reliable approach, balancing independent community work with team collaboration.
Qualifications and Requirements:
- Certificate III or IV in Community Services, Individual Support, Disability, Aged Care, or a related field (A Diploma in Community Services or equivalent is highly regarded).
- NDIS Worker Screening Check, Working with Children Check, and National Police Check (or willingness to obtain).
- Current First Aid and CPR certificate, or willingness to obtain.
- Right to work in Australia.
