About the Role:

We are currently seeking a significantly experienced practitioner to join our passionate and close-knit mental health team within South Eastern region of Melbourne. The NPS / COS programmes are national programs provided in partnership with ERMHA and aims to support people with severe and persistent mental illness and complex needs, including support for their carers and families.

As a Support Facilitator, you will work to provide recovery-focused, client-centred and care coordination. You will support eligible clients to develop a case plan based on their individual needs using a recovery-oriented approach. You will also facilitate services and support for clients, ensuring these services work in a more collaborative and integrated way including assistance with access to NDIS services and supports. A major focus of the role is to support hard to reach clients in an assertive outreach approach.

About you:

To be successful in this role, you must have a tertiary qualification in social work, psychology, mental health, Alcohol and Other Drugs or a related allied health professions. You will also have a comprehensive understanding of the Mental Health service system including clinical and non-clinical services within the region. Knowledge and experience around the NDIS (National Disability Insurance Scheme) will be highly regarded. You must also hold a valid Victorian driver's license.
